The prices below, in U.S. dollars (USD), are for PRO Clinic in Scottsdale, except the new-patient evaluation, which is the same at Family Orthopedics. The services priced below are cash-pay and are not billed to insurance; Medicare and virtually all private insurers classify regenerative injections as investigational. One time, at both clinics. A comprehensive history and exam, review of baseline labs and imaging, risks, benefits and alternatives, and your first written plan. At PRO Clinic, if a procedure is scheduled within 60 days, the fee is applied in full toward it.
Membership covers Dr. Johnson’s time, clinical judgment, monitoring and access throughout your protocol. It is not insurance — medications, labs, imaging and procedures are billed separately, so you always see exactly what you pay for. Membership begins only if Dr. Johnson determines therapy is right for you.

These procedures are investigational and not FDA-approved for treating musculoskeletal or orthopedic conditions. Fees for individual procedures include ultrasound or fluoroscopic guidance, physician time, consumables and one follow-up visit within 90 days. Members save 15% on PRP and BMAC.

Every infusion is physician-supervised, with screening and vitals before you start. Human research on IV NAD+ is still early, so we treat it as an optional wellness add-on.
The services priced above are cash-pay; not billed to insurance. HSA and FSA cards are accepted; whether they apply is up to your plan, so confirm eligibility with your plan administrator. Medical financing is available.

Regenerative procedures: 50% of the fee is due when you schedule, and the balance no later than 24 hours before treatment. The deposit is fully refundable if you cancel more than 5 business days ahead; within 5 business days it is non-refundable, because biologic products are ordered for each patient.
